Effective January 1, 2012, hospitals come under the provision of the Freedom of Information and Protection of Privacy Act (FIPPA). FIPPA is extended to cover hospitals as a result of the Broader Public Sector Accountability Act, 2010, which received Royal Assent on December 10, 2010.
The purposes of this Act are,
(a) to provide a right of access to information under the control of institutions in accordance with the principles that,
(i) information should be available to the public,
(ii) necessary exemptions from the right of access should be limited and specific, and
(iii) decisions on the disclosure of government information should be reviewed independently of government; and
(b) to protect the privacy of individuals with respect to personal information about themselves held by institutions and to provide individuals with a right of access to that information. R.S.O. 1990, c. F.31, s. 1.

The Kirkland and District Hospital promotes access to information and transparency of its governance and decision making. In many cases the information you seek may already be available, for example, on the hospital's website. Before making a formal request for access to information (or to correct your personal information) under the Act, contact the Privacy Coordinator and ask them if they are able to provide you with the information. If the information is readily available then they will direct you to the information, or where appropriate, send the information to you. By contrast, a Formal access request can take longer to process and may require payment of fees. (see fee schedule below)
When an informal request for access to information (or to correct your personal information) is not possible, you can make a formal request to the Freedom of Information and Protection of Privacy Office. You can access the request form by clicking on the link below. Although you are not required to use the provided form, we recommend that you do so to help ensure that your request is processed as quickly as possible. The hospital will not accept oral requests or requests via email.
Please note: If the Privacy Coorinator is unable to grant your informal request, making a formal request does not necessarily mean that you will be given access to the information. Each formal request will be processed and access may or may not be given based on the act.
