DISCHARGE PLANNING

The Kirkland and District Hospital Discharge Planning service ensures that patients who may have problems on discharge are appropriately identified. The patient's needs are coordinated by working with the patient, family, and health care team and community resources to plan and implement arrangements that best meet these needs following discharge.
